Dear Parents / Guardians,
Recent changes to the Idaho school immunization rules require that a child receive the following immunizations prior to admission into the 7th grade:
- One dose of Tdap booster, and
- One dose of Meningococcal vaccine.
Southwest District Health (SWDH) immunization program will be coming to your child’s school to hold an
immunization clinic for sixth, seventh and eighth graders on February 26, 2015. Permission slips and important information about these immunizations will be sent via Skyward Family Access email. The vaccines will be provided at no cost with a $20 per injection administration fee ($40.00 total for both vaccines). Please make checks out to Southwest District Health. If you have private insurance or Medicaid, SWDH will bill insurance or Medicaid and no payment will be required at the time of service. If you would like for your child to participate, please complete the forms and send the following to your child’s teacher before February 26, 2015.
- Completed permission slip
- Payment (if your child does not have insurance or Medicaid)
- Copy of insurance card or Medicaid card
- Your child’s shot record (so we can record the shots that are given)
