Backpack to School Assistance ProgramUpon application approval, backpacks and supplies can be picked up on August 16 at the Summer Alive Festival held at Hawthorn Park from Noon - 4:00pm OR August 17 at Canyon Springs Christian Church from 4:00pm - 5:30pm.

Proof of registration within the Middleton School District is required for pick up.

A registration receipt or class schedule is acceptable proof.

All fields are required. Up to four (4) students can be included on this form. Backpack to School Assistance Application

Your information on this form will be kept private and will not be shared with any third parties or anyone outside of the volunteers for this program; it is only for the purpose of making sure your child's needs are fulfilled as accurately as possible.

The Back-Pack to School Assistance Program is made possible by volunteers and community donations and support.


    Online Registration for Existing Students Begins July 1st

    All students of the Middleton School District need to register for school during the summer. Thanks to Family Access in Skyward, parents are able to register their children online and avoid the lines and their time restraints in August. Parents can update personal contact information (address, phone numbers, emergency contacts) mark permissions for field trips, publication and fluoride treatments, as well as pay registration fees.     Processing fees will be waived for online payments during the month of July.

    New Student Centralized Registration for All Schools August 12, 2014 @ Middleton Middle School

    Any new students to Middleton Schools and any family needing assistance with internet access should attend centralized registration. Centralized registration for K-12 (all schools) will be held at the Middleton Middle School, 511 West Main Street on August 12th (9 am-12 pm, 1-4 pm and 5-8 pm).

    Please bring the following information when you and your student come to register:
    1) Certified Birth Certificate (original),
    2) Immunization Records and
    3) Proof of Residency (a current utility bill, which includes a current address, or a notarized document that a home site has been purchased –the loan has closed- and construction has begun on the respective home within the Middleton School District boundaries).
